In this role, you will: Perform holistic assessments of clients within a community setting Identify and provide appropriate support interventions Recommend and arrange equipment or home adaptations to improve quality of life Collaborate closely with a multi-disciplinary team to ensure a seamless, person-centred care experience Maintain accurate and up-to-date records Requirements & Qualifications: To be successful in this role, you will need: A current and valid HCPC registration Proven experience in adult social care or community-based assessment Strong communication and interpersonal skills Ability to work both independently and as part of a multidisciplinary team A genuine commitment to improving the lives of vulnerable adults Working Hours & Contract Duration: This flexible locum position requires availability from Monday to Friday with some potential for hybrid working depending on the service’s needs.
