In this role, you will:? Perform comprehensive patient medication reviews ? Provide ward-based clinical pharmacy support for surgical & medical wards ? Deliver evidence-based treatments to optimise patient outcomes ? Collaborate with multidisciplinary teams to provide holistic care ? Maintain accurate patient recordsRequirements & Qualifications:To be successful in this role, you will need:? A current and valid GPhC registration ? Prior hospital ward experience and confidence in ward-based working ? Strong communication and interpersonal skills ? The ability to work independently and within a multidisciplinary team ? A commitment to delivering high-quality patient careWorking Hours & Contract Duration:This role offers a full-time position , Monday to Friday, 9 AM - 5 PM.
